At the Froedtert & the Medical College of Wisconsin health network, your privacy is important to us. This Website Privacy Policy (“Policy”) outlines the information gathering and dissemination practices we follow and your rights as a user of our website. This Policy applies to the information gathered or disseminated by Froedtert Health, Inc. and its affiliates including, but not limited to: Froedtert Memorial Lutheran Hospital, Inc., Community Memorial Hospital of Menomonee Falls, Inc., St. Joseph's Community Hospital of West Bend, Inc., Froedtert & the Medical College of Wisconsin Community Physicians, Inc., West Bend Surgery Center, LLC, Froedtert Surgery Center, LLC, and Inception Health, LLC.

Scope

This pledge applies to all information we collect in the United States through our website. For the purposes of this Policy, “websites” includes our mobile applications.

For our privacy practice relating to your protected health information, please refer to our Joint Notice of Privacy Practice located separately on our website.

What Information We Collect

When you visit our website, we may collect two types of information: personal information you provide if you interact with us in any way, including signing up for our newsletters or filling out a “Contact Us” form, and website-use data that is automatically collected as you browse our site.

Personal Information You Choose to Provide

If you choose to communicate with us, you will be asked to provide your name, address, phone number, e-mail address (under “contact information”) and other optional information, such as your age. Additionally, if you correspond with us by e-mail, we may also save the contents of your messages, as well as your e-mail address and our replies to your inquiries. Your answers to any surveys or questionnaires on our website might be retained too, so that we can adjust the information we provide or improve our services for you.

App Permissions

Depending on your device or Platform (as defined in our Terms and Conditions) permission settings, the Froedtert & MCW app may have access to the following information from your device:

  1. Location: Froedtert & MCW App collects location data to trigger notification upon arriving to clinic even when the app is closed or not in use. This data is also used to help scheduling Same-Day Care services.
  2. Write to external storage: Froedtert & MCW App may access your device's storage to playback educational videos. May access your device's storage to read and write files you choose to use in the application. These files may be used as file attachments that are sent to your provider or they may be created from file attachments sent to you from your provider. May store files uploaded from your device's storage in your medical record.
  3. Calendar: Froedtert & MCW App may use your calendar to save appointment details.
  4. Camera: Your camera will be used so your provider can see you during your visit. You can send photos to your care team if needed.
  5. Audio recording: Your microphone will be used so your provider can hear you during your visit.
  6. Phone state: Froedtert & MCW App may use your phone state to capture phone number, current cellular network information or status of ongoing calls.
  7. Bluetooth: a. Bluetooth will be used to request a connection, accepting a connection, and transferring data. b. Bluetooth will be used to connect to headphones or speakers, if you choose to use them. c. Bluetooth will be used to discover and pair local devices. d. Bluetooth will be used to connect to already paired devices.
  8. Wi-Fi state: Froedtert & MCW App may use your Wi-Fi state to turn off Wi-Fi network and use cellular one in order to gather necessary information.
  9. Other information: For example, Froedtert & MCW App may be able to receive data from the Internet, prevent your device from sleeping, control vibration, change your audio settings, view your network connections/state, obtain full network access, and run at startup.

These app permission settings allow you to access or use certain features within the App. For example, for telehealth services, the app may need access to your camera and microphone. By changing your app permission settings, you may affect your ability to access or use certain features on the app.

Additionally, you may be able to log into the App using your device's log in features such as using a passcode or your fingerprint. These device log in features are facilitated through your device and not FH and we have no control over such features, which you can change by accessing your device settings and preferences.

For Android Users - Required Google Play Disclosures for Certain Health Apps

Google has determined that our mobile app is subject to its COVID-19 app requirements. As a result, we are required to provide you the following information so we can make our mobile app available to you in the Google Play store.

  • Our mobile app interacts with your microphone only during telehealth visits and when you choose to use your microphone to navigate our mobile app. Our mobile app interacts with your camera only if you use your camera during telehealth visits, if you choose to send your care team images, or if you choose to add a profile image to your profile in our mobile app. This information isnot used in connection with COVID-19.
  • Our mobile app accesses, collects, uses, and shares your information (including location Information when you are at one of our facilities, video, audio, images, files, phone) as stated above in the section titled, “App Permissions.” We also prominently highlight these uses, describe the type of data being accessed, and obtain your consent for these purposes as you use our mobile app.
  • Our mobile app was not created specifically for the COVID-19 pandemic. It existed before the COVID-19 pandemic to allow you to access your health information. We may allow you to access COVID-19 related vaccination information, laboratory test results, and documents with illness-related information using our mobile app. You may choose if or how you want to access, display, or use the information - just like you can make those decisions about health information relating to other conditions, services, tests, or vaccinations.
  • We may allow you to use our mobile app to conduct telehealth appointments with your healthcare providers.

Website-Use Information

Along with the information you choose to provide us with, our website automatically collects information as you browse. Some examples include:

  • “Cookies,” which are small text files that a website or e-mail may place on your browser and store on your hard drive. These cookies allow us to recognize you as you use or return to our site. You may choose to browse our website without accepting cookies. Please note that cookies allow us to recognize you from page to page and, without cookies enabled, you may not be able to complete certain transactions on our website.
  • Web server logs collect data about how our site is being accessed and used. This technical information includes your internet protocol address, your device operating system, your browser type and the path you took to the site and during your visit.
  • Web beacons allow us to know if a certain page was visited, an e-mail was opened, or if ad banners on other sites were effective.

Location Information

To the extent that any of the services provided through the Platform require the use of your mobile location, we will request your consent before obtaining and using your mobile location. All mobile location data that we collect from you will be treated in accordance with our Privacy Policy as your personal information.

How We Use Your Personal Information

We recognize the importance of privacy. We do not sell or lease your personal information. We use your contact information to respond to your questions or to send you material and information you request, as well as to help us determine ways to restructure the website so it better serves you.

We may also use it to notify you of changes to our website or new services we think you may be interested in. You may elect not to receive these communications, either when you initially register as a member with our site or at any time thereafter.

We also use your website information in order to help identify problems with our website. We may use all information we collect to analyze statistical use patterns and demographic data, including where our visitors come from and what demographic characteristics they have and to improve our website to better serve our customers.

We participate in internet-based advertising. This means that we may use information about how you browse our website and others to help us improve our service offerings, websites and advertising.

We may share your personal information among our affiliates as necessary to provide you with the information and services you requested. Except in the limited circumstances listed, we do not provide unaffiliated third parties with personal information about our website visitors.

Such statistical data will not specifically identify you or any other user.

HealthKit and Google Fit

With your permission, certain versions of our Mobile Applications can connect to Apple HealthKit or Google Fit to receive health information and to share that information with your healthcare providers.

Our applications do not share your health information with HealthKit, Google Fit, or other software enabled with HealthKit or Google Fit.

Use of Cookies

Cookies are small bits of information generated by our web server and stored on your computer's hard drive. Cookies can remember what information a user accesses on a web page to simplify subsequent visits by the same user.

We use cookies to recognize you as a repeat visitor and to provide certain features that make your use of our website more efficient.

These cookies are not required to use the Web site in any manner and are only an option to make your browsing easier. If you would not like to receive cookies, you may decline by adjusting the browser settings on your computer.

Your Choices

You may exercise choices regarding our online operations and advertising. You may choose to browse our website without accepting cookies. Each web browser has settings allowing its users to turn off cookies.

It is very important to note that no one at Froedtert & the Medical College will ever ask you for your password.
